Accounts payable software for healthcare captures a vendor bill, routes it to the person who can approve it, and pays the supplier without dropping the entity, the department, or a patient-related code.

The ledger question comes first: a specialty clinic can keep QuickBooks or Xero and bolt a pay tool on top. A multi-site group that consolidates clinics, or that stores protected health information in the financial system, is buying the ledger and the payables workflow together.

Sage Intacct puts accounts payable inside core financials and says the product with Advanced Audit Trail is HIPAA compliant. AvidXchange leaves that ledger alone and sells AvidInvoice plus AvidPay on top, with NetSuite, Sage, and Yardi among the systems it names.

Matching is the hospital problem, not the scan of a PDF. A supply invoice has to meet the purchase order and the receipt. Medius puts multi-way matching in both AP packages. Stampli ships a semi-automated 2-way and 3-way match and sells full automation separately, so the quote has to name the match. One expense line, with no entity and no purchase order, forces a second coding pass before close.

Payment is a second meter beside the subscription. BILL publishes ACH, check, card, and wire fees. AvidXchange, Medius, and Stampli leave those terms on a quote.

Xero and Melio can cover a one-office practice. They are the wrong default once a second legal entity, a business associate agreement, or a purchase-order match is the job.

Why the entity and the patient data decide the tier

The demo usually shows a clean approval on a seat a clinic will not sign. On BILL, Essentials is $49 per user per month and pushes bills out by CSV, so QuickBooks Online and Xero go stale. Skip that seat when the department already lives in those books.

Team is the first BILL plan with automatic two-way sync to QuickBooks Online, plus QuickBooks Pro and QuickBooks Premier, along with Xero, and it lists the Invoice Coding Agent. Corporate adds custom approval policies, purchase orders, and an approver-only discount BILL never states in dollars.

Sync to Sage Intacct and to NetSuite starts on Enterprise, at custom pricing, with multi-entity accounting and 2-way and 3-way matching that BILL says varies by accounting software. A practice on Intacct that signs Team bought the wrong tier. On Essentials and Team every login is a full seat, including a department head who only approves. Five Team seats cost $325 a month before a single ACH fee.

Medius prices the step up by entity. AP Essentials is unlimited users, one sandbox, and one entity. AP 360 is unlimited users and three entities, plus Copilot, supplier conversations, fraud and risk detection, and a supplier portal. Both are quotes, and fraud detection is not on Essentials. Core financials include accounts payable, purchasing, and multi-entity consolidation. The HIPAA claim is the product with Advanced Audit Trail. Avertium certified the safeguards as HIPAA and HITECH compliant, Sage will sign a business associate agreement with eligible clients, and the pages carry HFMA Peer Reviewed designation. After the card is approved, Ramp's free plan ingests supplier invoices, sends each bill to an approver, and pays by ACH, a check, a card, or a wire, with QuickBooks Online and Xero. NetSuite and Sage Intacct begin on Plus. Workday and Oracle Fusion Cloud begin on Enterprise, which is custom, so a hospital ledger is not the free plan.   2. Decide whether vendor invoices can hold protected health information. If they can, start with a vendor that will sign a business associate agreement. Sage Intacct states that path.

  3. Count entities and approvers separately. Medius prices the jump from one entity to three, and BILL prices every approver on Team. Tipalti does not charge per user and still charges per transaction.

  4. Write a normal clinic month of ACH, checks, and card-funded supplier payments on its own line, then set that total beside the seat or the platform fee.
