A medical practice runs three systems that each hold part of the money. The practice management or EHR system holds charges, claims, remittances, and patient balances. The bank holds deposits: payer transfers and card settlements. The general ledger holds the books the CPA signs. Accounting software is that third system. Its job is to record cash and expenses and tie each deposit to the claims behind it without storing the patient detail itself.

A payer sends one transfer that covers many claims, with a remittance listing the patients. The practice system posts each claim. The ledger needs one line for the payer deposit, matched to the bank, with the remittance total and trace number as the memo.

The ten products below fall into three jobs. QuickBooks, Zoho Books, Sage Intacct, Xero, NetSuite, and FreshBooks are ledgers. Tebra, AdvancedMD, and SimplePractice run the chart, the claims, and the patient balance. Gusto runs payroll. None of the second or third group replaces the first.

Why the ledger and the practice system disagree

The expensive mistake is treating the practice system's revenue as the ledger's revenue. Charges are gross. The payer contract writes part of each charge off. The deposit is what remains. A ledger that books gross charges overstates income and creates a receivable nobody reconciles.

The compliance mistake is quieter. Intuit's support article says QuickBooks Online meets industry standards for online security but is not compliant with the HIPAA standards for privacy, and it recommends against entering individually identifiable health information. A memo line that pairs a patient name with a visit code puts that information into a file with no business associate agreement behind it. Vendors differ on the path to one: Zoho offers its template on request, Sage signs one with eligible accounts that subscribe to its Advanced Audit Trail module, and Oracle requires NetSuite customers to buy HIPAA for NetSuite and sign a BAA before storing ePHI.

The third problem is cadence. Xero's own healthcare guide says to reconcile insurance payments at least monthly, and weekly for higher-volume practices or payers with frequent discrepancies, so underpayments and denials surface before they compound.
